What is Vidalista 40 mg?

Vidalista 40 mg contains 40 mg of tadalafil, the same active ingredient as the branded medicine Cialis. It is produced by Centurion Laboratories Pvt. Ltd., an Indian manufacturer.

Tadalafil differs from sildenafil mainly in how long it stays in the body. Its half-life is about 17.5 hours, so one dose can support erections for up to 36 hours. What is Vidalista 40 mg used for?

Vidalista 40 mg is supplied for the treatment of erectile dysfunction (ED) – a persistent difficulty in getting or keeping an erection firm enough for sexual intercourse.

A note on strength: for ED, tadalafil is normally prescribed at 5 mg to 20 mg, and 20 mg is the highest dose in the standard prescribing information. The 40 mg strength is above that range. Higher doses raise the chance of side effects without a proven gain in effect, so 40 mg should be used only on a doctor’s specific advice. Most men start with Vidalista 10 mg or Vidalista 20 mg.

Tadalafil at 40 mg once daily is also an approved treatment for pulmonary arterial hypertension (PAH) under other brand names. PAH is a serious lung condition that must be managed by a specialist, not self-treated.

The enzyme PDE-5 breaks cGMP down. Tadalafil blocks PDE-5, so cGMP lasts longer and blood flow improves. Without sexual stimulation, nitric oxide is not released and tadalafil has no effect – it does not cause an erection on its own and does not change libido.

• When: At least 30 minutes before sexual activity. The effect can last up to 36 hours.
• How often: No more than once in 24 hours. Do not take a second tablet the same day.
• With water: Swallow the tablet whole.
• Food: Absorption is not affected by meals, so it can be taken with or without food.

Drug, disease and food interactions

Medicines that must not be combined

• Nitrates – nitroglycerin, isosorbide mononitrate or dinitrate, and amyl nitrite (“poppers”). The combination can drop blood pressure to a dangerous level. Because tadalafil stays in the body for a long time, at least 48 hours should pass after a dose before a nitrate is given, and only under medical supervision.
• Riociguat – used for pulmonary hypertension.
• Other PDE-5 inhibitors – sildenafil, vardenafil, avanafil or a second tadalafil product.

Medicines that need caution or a lower dose

• Alpha-blockers such as tamsulosin, doxazosin or terazosin
• Blood pressure medicines in general
• Strong CYP3A4 inhibitors: ketoconazole, itraconazole, ritonavir and other HIV protease inhibitors, clarithromycin
• CYP3A4 inducers such as rifampicin, which can make tadalafil less effective

Health conditions to discuss with your doctor

• Heart attack, stroke or a serious heart rhythm problem in the last 6 months
• Unstable angina, heart failure or low blood pressure (below 90/50 mmHg)
• Uncontrolled high blood pressure
• Kidney disease or dialysis
• Liver disease – tadalafil is not recommended in severe liver impairment
• Retinitis pigmentosa or previous sudden vision loss (NAION)
• Peyronie’s disease or another penile deformity
• Sickle cell anaemia, multiple myeloma or leukaemia

Food and alcohol

• Grapefruit and grapefruit juice raise tadalafil levels in the blood and should be avoided.

• Start lower: Because 40 mg is above the usual ED range, ask your doctor whether a 10 mg or 20 mg dose would be enough.
